Medical release forms from Dr. Basri's office hours for the OCYFL to use

Everyone, Dr. Basri has been able to accommodate everyone in the past that have issue's concerning Medical clearances. Here is his listing's, location's and addresses.

Please make sure that if you have players or Cheerleader's that need Medical clearances that they are taken care of prior to Aug. 1st, if they are not please consider him. They can not participant in any practice and it will then be your job to update us accordingly. No one can practice without one!
This is simple, no excuses for certain Insurance policies that don't line up with our dates, $25 is all it take's to satisfy the rules.

Here is a list of the offices where we can do medical clearances.

Parents should ask for appointments as we cannot accommodate walk ins.

The price per exam is $25. 

We take almost all insurances if the co pays are made at the time of visit.  We must be listed as either the Primary Care Physician (PCP) for that person or we get a referral from their own PCP.  Here are our office locations and phone numbers. Saturday hours are available in Montgomery.
